It is mandatory that all attendees of Rosina Andrews Events complete this waiver in advance or on the day before taking part in any of our events. By completing this form for myself or my child; I assume all the risks of participiating in all activities associated with this event. I confirm that the attendees are physically fit and have not been advised to not take part in dance based activities by any medical professional.
I hereby consent for myself, or child to receive medical treatment that may be deemed advisable in the event of injury, accident or illness during the event.
I consent to this event being photographed or videoed and agree to be included in this footage which may be used for promotional material.
